Case Management Services

Case Management Services are provided to the program participants by national CERI staff who have obtained a degree (Bachelor’s or Master’s Degree) in social work and include. Case Management Services are conducted through the following steps:

  • Initial and subsequent assessments are facilitated by CERI case managers
  • Results from these assessments are incorporated into a risk level determination that triggers service planning and a set of formal interventions delivered by the case manager or other professionals
  • Interventions delivered include referrals to receive medical treatment, provisions for their basic needs, counseling, provisions for emergency needs (shelter, food, clothing, transportation) and other
  • Monthly case reviews are conducted by the National Program Director to ensure that youth receive all needed services
